REQUIREMENTS:

PHOTOGRAPHS: Two 2" X 2" passport photos (color only)

PROOF OF CITIZENSHIP: Expired passport issued to an adult or Naturalization Paper or CERTIFIED COPY of your Birth Certificate (A certified copy will have a raised seal or colored seal and issued by Vital Statistics of the state in which you were born). If you were born in Alabama, you can now get a certified copy at the Madison County Health Department located on Max Luther Dr. in Huntsville. If you were born outside of Alabama, we have a phone list that will assist you with obtaining your birth certificate. If you are in possession of an expired adult passport, you may use this in lieu of the certified birth certificate.

If you do not have a certified copy of your birth certificate, you may CLICK HERE for information on how to contact the Bureau of Vital Statistics in the state where you were born.

PASSPORT FEE: Personal check, cashiers check or money order payable to U.S. Department. Of State. Must be separate checks for each application.

15 years & Under

Normal: $60.00 + $25.00
Expedite: $120.00 + $25.00
Overnight: $136.25 + $25.00

16 years & Over

Normal: $75.00 + $25.00
Expedite: $135.00 + $25.00
Overnight: $151.25 + $25.00

Passport Card Fees

15 years & Under

$10.00 + $25.00

16 years & Over

$20.00 + $25.00

* The $25.00 Execution Fee is not charged when adults 16 and over with a fully valid passport book apply for the passport card using a DS-82 form

 

Effective February 1, 2004 the DS-11 now requires ALL MINOR CHILDREN TO APPEAR IN PERSON
This change is made to further prevent international parental child abduction and child substitution

Both parents or legal guardian must appear to
sign the application in this office. If only one parent appears, you must also submit one of the following:

Second parent's written statement consenting to passport issuance for the child.
Primary evidence of sole authority to apply, OR
A written statement (made under penalty of perjury) explaining the second parent's unavailability.

* Requirements effective July 2, 2001 by Public Law 106-113, Section 236
implemented by the U. S. Department of State Passport Division.

 

EXECUTION FEE: $25.00 cash or check (personal check is acceptable) made payable to Jane Smith, Circuit Clerk.

IDENTIFICATION: A picture I. D. such as driver's license or military I. D.

Processing time is approximately 6 - 8 weeks from date of application

The total time is 3 weeks for expedited service.
