Affidavit of Residency (of a third party and must be notarized)
Divorce Decree
Separation Agreement (original and 2 copies)
** Child Support Information Sheet (form CS-47)
** Vital Statistics Form (Certificate of Divorce)

PARTIES WHO HAVE MINOR CHILDREN, THE FOLLOWING IS REQUIRED

** Child Support Obligation Income Statement/Affidavit (both parties)
** Child Support Guideline Form
** Child Support Guideline Notice of Compliance
** Withholding Order Payment of Child Support (form #CS-34 Instructions for Employer )
** Standing Pre-Trial Order ( "Transition in Parenting" Class )
** Children Cope with Divorce Seminar Certificate (both parties)

Effective January 1, 1994, when any order of Child Support is entered, the Withholding Order payment of Child Support shall be served immediately, unless the parties have reached an alternative agreement and the judge approves such agreement. The following forms must be completed and filed when the Withholding Order payment of child support is served immediately:

** 1) Notice to Defendant
** 2) Instructions for Employer
** 3) Answer to Order of Withholding
** 4) Order of Service and Return

Service fee for an immediate Withholding Order is $20 by sheriff or $5.49 by certified mail.

FILING FEE

Court Cost for filing of Divorce is $279.00, payable only by cash, money order or cashier's check. Filing Fee's are non-refundable.

The Court Cost for filing of Divorce is $279.00 (if being served by the Madison County Sheriff's Department- $299.00) - payable only by cash, money order or cashier's check. Filing Fee's are non-refundable.

DOCUMENTS REQUIRED FOR FILING

1. A letter or other document requesting registration.

2. Two (2) copies, including one certified copy, of the determination to be registered.

3. Statement under penalty of perjury that the order has not been modified.

FILING FEE

Court cost for filing is $382.00, payable by cash, money order or cashier's check.
